Loss Run Release Form Creation

Use the coverage matrix to edit policy terms, LOC's and Policy # (prefills based on WC Carrier)

Verify business address and contact for signature

Carrier Groups with multiple contacts can be selected in the appropriate drop down or you can add your own

This will generate the release forms in a PDF file, which can be emailed or faxed to insured for signature
